Living Will, Advance Decision
Description
An advance decision to refuse treatment lets your healthcare team and family know your wishes if you are not able to communicate them.
We will not require any medical information except whether you want to be an organ donor or not.
The allowance of pain treatment is accepted.
What will you get?
The final product will be a document that will be emailed to you in an editable format such as "Apache", Notepad or email, We will send a PDF on request.
We will create an advance decision document with a header page that applies in the UK but will work in many english speaking countries.
You will need to provide basic information names and addresses etc which you will need to email us, we will email the questions to you.
Printing, Witnesses and Signatures are down to you, we will type the advance decision.
